Preparation for Joint replacement surgery


This information is intended to be an overview of activities that you may experience during joint replacement surgery. It is not intended to replace any instructions provided by your doctor and we would encourage you to discuss this information with your doctor.

Four weeks before the surgery

Become familiar with your joint anatomy. You may find it useful to understand your joint anatomy and able to read answers to common questions. Check out guide knee surgery and guide to shoulder surgery. Start gentle exercise to strengthen your muscles.

Three weeks before the surgery

Store frequently used items in easy to reach cabinets, such as cleaning supplies. Avoid very high or very low shelves as these may require you to use a step stool or kneel.

Check the safety of your home to prevent falls or tripping

Move long electrical & telephone cords against the wall
Place a non-skin mat in your bath area
Make preparation to reduce climbing stairs


Two weeks before the surgery

Anti-inflammatory medications: Your doctor may not want you to take any non-inflammatory medications (pain-killers) 14 days prior to the surgery. Be sure to discuss this with your doctor.

One week before the surgery

Make note of all the pharmaceuticals ,medication you take, the dosage and how often you will want to take this with you when you are pre-admited at the hospital.

Week of the surgery

What to bring to the hospital: Below is a list of things you may want to bring with you to the hospital I preparation for your surgery. Talk to your doctors he may have additional information about preparing for your hospital stay.




· Do not bring your personal belongings until after surgery. Tell your family that your room will be assigned when you are in surgery or in recovery and which point they can bring your personal items to your room.
· Personal grooming items that you may want to pack include a toothbrush, toothpaste, hairbrush, eyeglasses/contacts, comb, shaving cream / razor, shampoo, undergarments, etc.
· Bring slippers to walk I the hallway.
· Bring loose fitting clothes for your trip home.

On arrival at the hospital.

How you will feel after the surgery: Be sure to ask your physician how you may feel after the surgery. You may feel nauseated, dizzy, weak, experience loss of appetite, have constipation and slight pain. Patients usually recover in few days after surgery.

Self-care after the surgery: A catheter may be in place after the surgery. It is normally removed when you can get out of bed to the commode or restroom; you can feed yourself, and can bathe your face and upper body. Your bandage will probably be changed once or twice a day.

Visitors after the surgery: you are allowed visitors once you have been moved from the recovery room into your room/ward. If you would prefer not to have visitors, please tell then nursing staff so they can communicate this to the visitors.







Your stay at the hospital:

Your surgeon will visit you and the physiotherapist will begin post-operative exercises. Your pain may be managed with injection or pills.

Self-care:

Our care takers will help you to walk to the wash room till you are able to support yourself. Walking to the restroom will build your strength & prepare you for discharge to home. You can bathe yourself with a little help, around your surgery area.

Discharge from the hospital:

You will discharged from the hospital when your doctor feels the time is right, this is usually five days (less or more depending on the type of surgery) after surgery. You will need to make arrangements for someone to drive you home from the hospital, as you will not be able ton operate an automobile. You will also be asked to wear comfortable, loose clothing to avoid pressure on the incision area.

Transfer to a rehabilitation facility:

Depending on your situation, your doctor may recommend that you be transferred to rehabilitation facility after you are discharged from the hospital. You may require few more day so you can begin physiotherapy & your health can monitored.

Fill pain medication prescription:

Your surgeon will probably give you a prescription for pain. You will probably need less each day but you may need them at night for a few days. Check with your doctor. Do not drink alcoholic beverages while you are taking pain killers.

Once you are home

It is very important that you follow your doctor’s instructions. The following suggestions should be discussed with your surgeon before your hospital discharge.

If you will be using walker or crutches to assist with walking, ask your doctor how much weight you may put on your operated joint.
Remember that you will probably tire more easily than usual. You may want to plan a rest period of 30 to 60 minutes mid-morning and mid-afternoon.
It is safer and easier to get in and out of chairs using both arm sand you should avoid low or overstuffed furniture. To increase your comfort, use a cushion or pillow to raise your body while seated.

Constipations is a common problem for patients following surgery. This is usually due to your limited activity and medications you may be taking. Discuss your diet with your doctor. It should include fresh fruits and vegetables as well as eight full glasses of liquid each day, unless your doctor tells you otherwise.
Your doctor will probably give you a prescription for pain medication. Please follow your doctor’s instructions concerning these medications.
Some swelling around the incision is normal. You will find it more comfortable to wear loose clothing to avoid pressure on the incision. Ask your doctor or other qualified health professional about appropriate wound care.
Eat well-balanced meals for healing purpose.
Wound care-in some cases, when you are discharged your bandage will be removed and may be replaced with a smaller bandage. If your post-operative appointment has not already been scheduled, call your doctor after a day or two after you are home from the hospital to set up your post-operative visit, which is typically two weeks after your surgery.

Three weeks after the surgery

Post-operative appointment. Your surgeon will examine the surgery area to check for any signs of infection. Your stitches / staples will likely be removed as well. Your range of motion will also be examined and you will review your physical therapy instructions. You will want to wear comfortable, loose clothing. You will likely return for follow-up appointments at three months, six months, and one year post-operation. You will probably return once a year thereafter.

This shows that the physiological basis for empathy, is defined as the ability to understand and share the experience of another person. Now research at the University of Wisconsin has used advanced brain imaging (fMRI, functional magnetic resonance imaging) to show that compassion meditation, a particular form of Buddhist meditation can increase the human capacity for empathy.

According to researchers, because of the small size of this subgroup, these data should be considered preliminary and must be confirmed in further studies.
For a subset of participants with a softer, glucosamine and chondroitin sulfate together or alone did not provide statistically significant pain relief.

Who does not like to smell nice? Well, I'm here to tell you that if you want to smell nice, use a little perfume or cologne .... or any bodyspray. Your skin care products should not contain fragrances. Many fragrances are produced from ingredients that are known to be toxic or carcinogenic.
You must remember that your skin is the largest organ of the body. Anything you put on it can be easily absorbed through the pores. Now, why I recommend you use perfume or cologne as a fragrance but to choose skin care product without them?
The reason is simple. When you use skin care products, they apply across the face, neck and body. It covers many of the surface area and, consequently, a large chemical absorption.
Now, I am sure that many colognes and perfumes contain less than desirable substances too, but I am realist enough to know that people are not going to stop using these products to smell nice. However, perfumes and colognes, you can achieve what you want from a small DAB here and there.
Thus, in my opinion, it is much faster than going to use skin care products that also contain potentially dangerous chemicals, aroma and massage are applied directly to the skin throughout the body.
Parabens: methyl, propyl, butyl and ethyl paraben. Many skin care (and moisturizing products) will use parabens as a preservative so their products have a long shelf life. The reason was purely economic. Nevertheless, studies have shown that they can cause cancer and interfere with the body of the endocrine system and cause allergic reactions and skin rashes.
Alcohols: ethanol, ethyl alcohol, methanol, benzyl alcohol, isopropyl alcohol and SD alcohol. Not all alcohols have the same properties, but those that are commonly found in skin care products are very drying and irritating the skin. Alcohols such as these strip away the natural acid mantle of the skin that makes you more vulnerable to bacteria, mold and viruses.
Now, when we looked at some bad, let's look at some of the good ingredients and some ingredients that sound good, but really are not.

Tai chi may be a safe alternative to traditional exercises for maintaining bone mineral density (BMD) in postmenopausal women. Bone mineral density is a key indicator of bone strength. Low bone mineral density is associated with osteoporosis, a bone disease characterized by reduced bone strength that can lead to fractures, which are a major cause of disability among the elderly.

Exercise is an important component of osteoporosis prevention and treatment. Tai Chi is a practice of mind-body which originated in China as a martial art. It consists of body movements slow and gentle, while breathing deeply and meditating (tai chi is sometimes called "meditation in motion").

Peter Wayne, Ph.D., and colleagues conducted a systematic review of research on the effect of tai chi on BMD. They found that tai chi may be an effective, safe and practical intervention for maintaining BMD in postmenopausal women. They note that the evidence is preliminary because they considered the research was limited in scope and quality, but enough evidence of effectiveness exists to warrant further research.

The authors further note that the benefits of tai chi appear similar to those of traditional exercises. However, tai chi may also improve balance, reduce the frequency of falls, and increase the strength of the musculoskeletal system.

About Menopause

A woman said to have completed natural menopause when she has not had a period of 12 consecutive months. For American women, this generally occurs around age 51 or 52. Common symptoms during the menopausal transition

Some women experience symptoms that are related to menopause and decreased activity of the ovaries. Others may be related to aging in general.

The scientific evidence of a link with the menopause is strongest for the following symptoms:

* Hot flashes and night sweats (also called vasomotor symptoms, because it involves the dilation of blood vessels)
* Sleep problems

Hormonal replacement therapy for menopause symptoms

For decades, hormone replacement therapy (HRT), most recently known as hormone therapy for menopause (MHT), was the main treatment of conventional medicine for menopausal symptoms. In 2002, the results of a large study called the Women's Health Initiative raised serious concerns about the long-term safety of MHT.

Black cohosh (Actaea racemosa, Cimicifuga racemosa). This herb has received more scientific attention for its possible effects on menopausal symptoms than have other plants. Studies on its effectiveness in reducing hot flashes have had mixed results. A study funded by NCCAM and the National Institute on Aging found that black cohosh, used alone or with other plants, failed to relieve hot flashes and night sweats in postmenopausal women or those approaching menopause. Other research suggests that black cohosh does not act like estrogen once was thought.

United States Pharmacopeia Experts recommend that women should stop using black cohosh and consult alternative health care if they have a liver disorder or symptoms of liver problems such as abdominal pain, dark urine or jaundice . There have been several reports of cases of hepatitis (liver inflammation), as well as liver failure among women taking the black cohosh. It is not known if black cohosh was responsible for these problems. Although such cases are very rare, and the evidence is not definitive, scientists are worried about the possible effects of black cohosh on the liver.
* Dong Quai (Angelica sinensis). One randomized clinical study of Dong Quai has been done. Researchers do not have to be useful in reducing hot flashes. Dong Quai is known to interact with and increase activity in the body, blood-thinning medicine warfarin. This may cause bleeding complications among women taking the drug.
* Ginseng (Panax ginseng or Panax quinquefolius). The panel concluded that ginseng may help with certain symptoms of menopause symptoms such as mood and sleep disorders, and his overall sense of well-being. However, it was not considered useful for hot flashes.
* Kava (Piper methysticum). Kava may decrease anxiety, but there is no evidence that it reduces hot flashes. It is important to note that kava has been associated with liver disease. The FDA issued a warning for patients and providers about kava because of its potential to damage the liver.

Such a request must be followed by the words "This statement has not been evaluated by the Food and Drug Administration. This product is not intended to diagnose, treat, cure or prevent any disease. Effectiveness of Four Weight-loss Plans
The very low carbohydrate diet known as the Atkins diet may contribute to greater weight loss than higher carbohydrate plans without negative effects such as increased cholesterol. Christopher Gardner, Ph.D., and colleagues at Stanford University conducted an NCCAM-funded study of 311 pre-menopausal women, all of whom were overweight or obese. Each woman was randomly assigned to one of four diets. Each of the diets used were selected for their different levels of carbohydrate consumption.

The Atkins diet is very low in carbohydrate consumption: less than 20 grams of carbohydrates per day and increasing to 50 grams per day.
The Zone diet is designed so that a person's daily calorie consumption is comprised of 40 percent carbohydrates, 30 percent protein, and 30 percent fat.
The LEARN diet (Lifestyle, Exercise, Attitudes, Relationships, and Nutrition) instructs participants to get 55 to 60 percent of their calories from carbohydrates, and not more than 10 percent from saturated fat. This diet is based on the USDA food pyramid.
The Ornish diet's primary guideline states that participants should not get more than 10 percent of their calories from fat.

Participants in each group received books that accompanied their assigned diet plan, and attended hour-long classes with a registered dietitian once a week for the first 8 weeks. Data on the participants was collected at the beginning of the study, and at 2, 6, and 12 months. The researchers recorded body mass index (BMI); percent body fat; waist-hip ratio; as well as metabolic measures such as, insulin, cholesterol, glucose, triglyceride, and blood pressure levels.

The Atkins diet group reported the most weight loss at 12 months with an average loss of 4.7 kilograms, or just over 10 pounds. They also had more favorable overall metabolic effects. Average weight loss across all four groups ranged from 3.5 to 10.4 pounds. What the Science Says About Acupuncture for Pain


Acupuncture treatment, among the oldest healing practices in the world, is part of traditional Chinese medicine. Acupuncture practitioners stimulate specific points on the body—most often by inserting narrow needles through the skin. In traditional Chinese medicine theory, this regulates the flow of qi (vital energy) along pathways known as meridians.

Acupuncture treatment has been studied for a wide range of pain conditions, such as postoperative dental pain, carpal tunnel syndrome, fibromyalgia, headache, low-back pain, menstrual cramps, myofascial pain, osteoarthritis, & tennis elbow.

Overall, it can be difficult to compare acupuncture research results from study to study & to draw conclusions from the cumulative body of evidence. This is because studies may use different acupuncture techniques (e.g., electrical vs. manual), controls (comparison groups), & outcome measures.

Four complex factor in acupuncture research is choosing the controls for a clinical trial. The choice depends in part on whether the researchers need to study a particular aspect of acupuncture (e.g., effects on the brain) or to decide whether acupuncture is useful compared with other forms of care. Examples of control groups include study participants who get no acupuncture, simulated acupuncture (procedures that mimic acupuncture, sometimes also referred to as "placebo" or "sham"), or other treatments (in addition to or in place of acupuncture or simulated acupuncture).

An emerging theme in acupuncture research is the role of the placebo. For example, a 2009 systematic review of research on the pain-relieving effects of acupuncture compared with placebo (simulated) or no acupuncture was inconclusive. The reviewers found a tiny difference between acupuncture & placebo & a moderate difference between placebo & no acupuncture; the effect of placebo acupuncture varied considerably, & the effect of acupuncture appeared unrelated to the specific kind of placebo procedure used. All of the study participants received standard care, typically consisting of analgesic drugs & physical therapy.
* Carpal tunnel syndrome—Although a 1997 NIH consensus statement on acupuncture concluded that acupuncture was promising for carpal tunnel syndrome, additional research confirming acupuncture's efficacy for this condition is scant.
* Fibromyalgia—Evidence on acupuncture for fibromyalgia is mixed. Some reviews of the scientific literature have found the evidence promising. However, another review that focused on the few rigorous randomized controlled trials on acupuncture as an adjunct therapy for fibromyalgia did not find a benefit. Additionally, a 2003 assessment by the Agency for Healthcare Research & Quality concluded that the evidence was insufficient & the beneficial effects of acupuncture for fibromyalgia could not be determined.
* Headache/migraine—Study results on acupuncture for headache are conflicting. Some literature reviews found evidence to support the use of acupuncture for headache, but others noted that most of the studies were of poor quality. A 2008 review of randomized trials on acupuncture highlighted a few well-designed trials whose findings indicate that acupuncture reduces migraine symptoms & is as effective as headache medications. In addition, a 2009 review found that acupuncture may help relieve tension headaches. However, four large trials that looked at acupuncture for migraines found no difference between actual & simulated acupuncture, both of which were equal to conventional care or superior to no treatment.
* Low-back pain—According to clinical practice guidelines issued by the American Pain Society & the American College of Physicians in 2007, acupuncture is four of several therapies physicians should consider when patients with chronic low-back pain do not respond to conventional treatment. In early, tiny studies, combining actual acupuncture with conventional treatment was more effective than conventional treatment alone for relieving chronic low-back pain; but actual acupuncture was not more effective than simulated acupuncture or conventional treatment. However, a large, rigorously designed clinical trial reported in May 2009 found that actual acupuncture & simulated acupuncture were equally effective—and both were more effective than conventional treatment—for relieving chronic low-back pain. There is insufficient evidence to draw definite conclusions about the effectiveness of acupuncture for acute low-back pain.
* Menstrual cramps—Two literature reviews have suggested that acupuncture may help with pain from menstrual cramps, but the research is limited.
* Myofascial pain—The evidence for acupuncture & myofascial pain (in which pain occurs in sensitive areas, known as trigger points, in the muscles) is mixed. Some literature reviews have found the evidence promising, but another review indicated that "needling therapies" for myofascial trigger point pain were not more effective than placebo.
* Neck pain—Studies of acupuncture for chronic neck pain have found that acupuncture provided better pain relief than some simulated treatments. However, the studies varied in terms of design & most had tiny sample sizes.
* Osteoarthritis/knee pain—Acupuncture appears to be effective for osteoarthritis, in the area of knee pain. Recent literature reviews have found that acupuncture provides pain relief & improves function for people with osteoarthritis of the knee. However, authors of a 2007 systematic literature review suggested that although some large, high-quality trials have shown that acupuncture may be effective for osteoarthritis of the knee, differences in the design, size, & protocol of the studies make it hard to draw any definite conclusions from the body of research. These authors concluded that it is soon to recommend acupuncture as a system part of care for patients with osteoarthritis.
* Postoperative dental pain—Although recent data on acupuncture for postoperative dental pain are scant, literature reviews based on earlier evidence have identified acupuncture as a promising treatment for dental pain—especially pain following tooth extraction. For example, a 1999 study of 39 dental surgery patients found that acupuncture was superior to placebo (simulated acupuncture) in preventing postoperative pain. However, a 2005 study of 200 dental surgery patients found no significant analgesic effect for acupuncture compared to simulated acupuncture, although patients who believed they received acupuncture reported significantly less pain than those who believed they received a placebo.
* Tennis elbow—Study results on the use of acupuncture for tennis elbow (lateral epicondyle) pain are mixed. An early review of clinical trials reported that data on acupuncture for lateral epicondyle pain were insufficient & of poor quality; however, recent reviews have found the evidence promising, noting strong evidence that acupuncture provides short-term pain relief for lateral epicondyle pain.

There is evidence that people's attitudes about acupuncture can affect outcomes. In a 2007 study, researchers analyzed data from four clinical trials of acupuncture for various types of chronic pain. Participants had been asked whether they expected acupuncture to help their pain. In all four trials, those with positive expectations reported significantly greater pain relief.

Acupuncture treatment has also been studied for a variety of other pain conditions, including arm & shoulder pain, pregnancy-related pelvic & back pain, & temporomandibular joint (jaw) dysfunction. Although some studies have produced some positive results, more evidence is needed to decide the efficacy of acupuncture for any of these conditions.

In addition to studying acupuncture's efficacy, researchers are looking at potential biomechanisms—that is, how acupuncture might work to relieve pain. There's several theories about these biomechanisms (e.g., acupuncture activates opioid systems in the brain that respond to pain); additional research is still needed to check the theories. Researchers are using neuroimaging techniques such as functional magnetic resonance imaging (fMRI) to look at the effects of acupuncture on various regions of the brain.
Side Effects & Risks

Acupuncture treatment is generally considered safe when performed by an experienced practitioner using sterile needles. Relatively few complications from acupuncture have been reported. Serious adverse events related to acupuncture are rare, but include infections & punctured organs. Additionally, there's fewer adverse effects associated with acupuncture than with plenty of standard drug treatments (such as anti-inflammatory medication & steroid injections) used to manage painful musculoskeletal conditions like fibromyalgia, myofascial pain, osteoarthritis, & tennis elbow.

Acupuncture Side Effects & Risks

Relatively few complications from the use of acupuncture have been reported to the FDA, in light of the millions of people treated each year & the number of acupuncture needles used. Still, complications have resulted from inadequate sterilization of needles & from improper delivery of treatments. Practitioners should use a new set of disposable needles taken from a sealed package for each patient & should swab treatment sites with alcohol or another disinfectant before inserting needles. When not delivered properly, acupuncture can cause serious adverse effects, including infections & punctured organs.
Status of Acupuncture Research

The U.S. Food & Drug Administration (FDA) regulates acupuncture needles for use by licensed practitioners, requiring that needles be manufactured & labelled according to definite standards of pharmaceuticals. For example, the FDA requires that needles be sterile, nontoxic, & labelled for single use by qualified practitioners only.

There's been plenty of studies on acupuncture's potential health benefits for a wide range of conditions. Summarizing earlier research, the 1997 NIH Consensus Statement on Acupuncture found that, overall, results were hard to interpret because of problems with the size & design of the studies.
Some recent studies have looked at:
* Whether acupuncture works for specific health conditions such as chronic low-back pain, headache, & osteoarthritis of the knee
* How acupuncture might work, such as what happens in the brain during acupuncture treatment
* Ways to better identify & understand the potential neurological properties of meridians & acupuncture points
* Methods & instruments for improving the quality of acupuncture research
Finding a Qualified Practitioner

Health care providers can be a resource for referral to acupuncturists, & some conventional medical practitioners—including physicians & dentists—practice acupuncture. In addition, national acupuncture organizations (which can be found through libraries or Web search engines) may provide referrals to acupuncturists.

* Check a practitioner's credentials. Most states need a license to practice acupuncture; however, education & training standards & requirements for obtaining a license to practice vary from state to state. Although a license does not ensure quality of care, it does indicate that the practitioner meets definite standards regarding the knowledge & use of acupuncture.
* Do not rely on a diagnosis of disease by an acupuncture practitioner who does not have substantial conventional medicine
